We’re excited to learn about your swimmer! After reviewing this form, we’ll schedule a brief consultation to discuss your family’s needs and answer questions. For everyone’s privacy and safety, facility details are shared after consultation, identity verification, and enrollment approval. We’ll then send a detailed intake packet to help us personalize your swimmer’s experience.
